Case
Prof. Miquel Quer (Bareclona)

A 75-year-old female presents with dysphagia related to a familial form of oculopharyngeal muscular dystrophy. She has no other prior medical history. Her entire maternal family is affected by this genetic disorder, which causes progressive muscle weakness.

The patient first experienced dysphagia 25 years ago, with a gradual worsening over time. Initially, the dysphagia affected only solid foods.

Currently, her condition has progressed to the point where she can no longer consume solid food and experiences frequent coughing during meals. Eating requires one hour or more to complete, and she reports a moderate weight loss (5 kg in one year). In the past two years, she has also suffered from recurrent respiratory infections.

Examinations:
- FEES: Residue in the hypopharynx; penetration; reduced propulsion.
- Videofluoroscopy: Residue in the hypopharynx, with a lack of cricopharyngeal muscle relaxation.

Proposed surgery: External cricopharyngeal myotomy.
